Different approaches to depression

Antidepressants act through biological mechanisms that vary by medication. TMS uses magnetic pulses to stimulate brain circuits involved in mood. Both can be useful, and choosing an option depends on your diagnosis, past response, safety considerations, and preferences.

When medication is not enough

Some patients have an incomplete response, while others find side effects difficult to tolerate. A clinician first reviews whether prior trials were adequate and whether another medication, psychotherapy, augmentation, or TMS is appropriate.

TMS can be added to ongoing care

TMS does not necessarily replace medication. Many patients continue prescriptions and psychotherapy while receiving treatment. Your prescribing clinician manages medication changes; do not reduce or stop them on your own.

Compare the practical commitments

Medication is generally taken at home and requires follow-up. TMS requires repeated office visits, screening, and monitoring. TMS has no routine sedation, but scalp discomfort, headache, and rare serious risks still need discussion. Avoid thinking of either approach as free of risks.

Ask what success would look like

Discuss the symptoms you most want to improve, how progress will be tracked, expected costs, and what will happen if benefit is limited. A treatment plan may use more than one approach rather than an either-or choice.
